Ministry Support (Diakon Lutheran Social Ministries) LPN Care Manager (Skilled Nursing) in Topton, Pennsylvania

Category/Shift/Hours per 2 week pay period: Full Time / Night Shift / 80 hours per pay period Diakon Lutheran Social Ministries is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Senior Living Services and Diakon Child, Family, and Community Ministries.General Responsibilities: *Responsible for planning, organizing and directing all operations of the assigned facility with the support of an on-call RN, as well as being responsible for performing a variety of duties to provide quality nursing care to residents, and to coordinate total nursing care for residents. *Performs rounds according to established schedule; documents resident progress, changes in condition, etc.; notifies RN if any change in resident's condition. *Oversees the overall clinical care being provided to residents; insures a high degree of quality resident care is maintained at all times. *Education/Training: Current State license to practice as a Licensed Practical Nurse required. Experience: A minimum of one (1) year experience in working as a charge nurse in a long-term care geriatric setting preferred or at least three (3) years working in a skilled nursing setting normally required. ADA: Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 50 lbs.; visual and auditory skills.Compensation and Benefits: Diakon offers a competitive compensation and benefits package for Full-Time and Part-Time employees.
