At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health and other employed team members.
The Telephonic Nurse Practitioner position will be responsible for afterhours telephonic care and triage, including dispatching on-call providers in market to sustain the Landmark promise to its patients of 24/7 in-home urgent care. This position will be a remote, home-based role.
Responsibilities
Provide superlative and empathetic telephonic care for Landmark patients
Actively listen and probe patients in a professional and timely manner to assess and evaluate patient’s clinical need
As appropriate, problem solve patient concerns telephonically
Communicate to and dispatch in-market on-call providers when issues cannot be solved telephonically
Document patient interactions in EMR
Handle both inbound and outbound calls as needed
Communicate and collaborate with members of the Landmark interdisciplinary team
Utilize critical thinking and effective communications to help connect the value of the Landmark program to patients in need of more information
Prescribe appropriate diagnostics and interventions to avoid unnecessary transfer/acute admissions
Responsible for patient follow-up to help avoid unnecessary medical costs
Ad hoc reporting may be required
Employees in the position can expect to be on the phone up to 90% of their work shift and complete other essential function on an as-needed basis
Full time and part-time roles available, with flexibility for changing shift coverage over time. Shifts are most mostly outside of normal business hours: first shift (6 am - 11 am), third shift (5 pm – 1 am), and weekends (9 am – 11 pm).
Qualifications
Completion of Nurse Practitioner accredited program
Minimum of two years clinical experience; experience in home-based care preferred
Minimum of two years of telephonic patient management
Knowledge of CPT, ICD-10 and HCPC codes
Thorough understanding of clinical standards of care
Experience dealing with UM standards, NCQA requirements, CMS guidelines, Milliman guidelines; understanding of Medicare, Medicaid, and Commercial populations preferred
Current unrestricted NP license
Fluency with Microsoft Office Suite; typing speed of 40+ WPM preferred
Home internet connection
Must be board certified at the time of hire or able to obtain board certification during the eligible timeframe (5 years following completion of residency or other training)
