Reference #: 9246624The Home Infusion Service Nurse (Home Visit Nurse) is a licensed professional who plans for, coordinates and provides individualized nursing care to patients and families in their homes. The provision of home infusion services reflect the mission, philosophy and standards of home infusion therapy care of the agency. This position reports directly to the Nurse Manager of the Department.Performs patient care in a timely and efficient manner. Meets productivity expectations. Maintains a positive impact on customers (internal and external)Reviews and revises plans of treatment, performs patient assessments as required by patient condition and or by agency policy. Coordinates the discontinuation of IV services with clients in a complete, thorough and timely manner. Paperwork is submitted for data entry within the 48 hour guidelineCommunicates with other members of the health care team on issues pertaining to client care needs, status updates and coordination of care issuesInteracts with co-workers, visitors, and other staff consistent with the core values of the Hospital.EDUCATION: ASN or BSN requiredEXPERIENCE: At least 2 years in the acute care setting or 1 year in home care, hospice, or home infusion settingLICENSES/CERTIFICATIONS:Valid PA RN license requiredBLS certification requiredValid PA driver's license requiredChemotherapy certification recommendedJefferson Health delivers state of the art healthcare services to patients throughout the Delaware Valley and southern New Jersey.
