The Care Coordinator RN or LPN will work closely with RN Care Advisor to coordinate collaborative patient care, case management and/or coordination experience in a setting that includes a collaborative, iterative process of assessing, diagnosing, planning to achieve mutually agreed upon patient goals, implementing interventions, and evaluating progress toward goals.

Where you fit in

This role is a part time RN or LPN role to provide remote home patient monitoring. The position is Monday – Friday, 8:00am – 5:00pm and 1 weekend per quarter. The position will be a division of our Community Pro-Care at Home team and will monitor our home O2 program patients.

Your exceptional qualifications

LPN; Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience. (Required)

Or

RN; Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), or Accreditation Commission for Education in Nursing (ACEN) accredited school of nursing or in lieu of accredited schools listed above, three years of related professional nursing experience. (Required)
