Cape Abilities Developmental Specialist/Case Manager in Eastham, Massachusetts

GENERAL SUMMARY Provides support & assistance to program participants in the areas of independent living, self-management within the community, social & independent living skills development, as well as affective & behavioral management. Communicates with families & providers, as required. Writes & manages program service plans & quarterly reviews in accordance with individual?s goals & regulation requirements. ESSENTIAL JOB FUNCTIONS* Establishes & maintains a rapport with day habilitation program participants. Ensures & safeguards members? human dignity, rights & liberties. Plans & oversees participant services in accordance with Day Habilitation regulations in the areas of independent living, self-management within the community, social & independent living skills development & affective & behavioral management. Assists program participants in making & expressing choices based on assessments & other service need indicators. Assists in the creation of goals & objectives. Plans program activities & supervises implementation of services created by the Developmental Specialists & interdisciplinary team (IDT). Assists in the development & expansion of opportunities for community integration. Attends participants ISP meetings as needed. Responsible for participant?s data & documentation as assigned. Provides physical assistance to program participants, as needed, including, but not limited to, toileting, transferring & feeding. Adheres to all funding source regulations & Agency policies & procedures. Establishes & maintains professional relationships with community contacts, colleagues, management & Department of Developmental Services (DDS) service coordination staff. Writes & oversees program service plans & quarterly reviews for each individual within regulatory requirements. In conjunction with interdisciplinary team (IDT), creates goals & objectives for all new individuals within 90 days of admission that cover habilitation, medical, social, occupational, & psychological needs. Designs service plan based on Service Needs Assessment (SNA) & defines specific goals based on SNA recommendations. Prepares quarterly reviews by summarizing daily & monthly documentation Defines behaviorally stated & measurable objectives that outline steps to be taken to reach the goals outlined in the Provider Support Strategies (PSS). Assists the IDT in planning the frequency & duration for which the services under the service plan will be provided. Identifies which integrated activities & therapies to include to successfully implement measurable goals within established timelines & in planning program activities that align with those goals. Serves as mandated reporter for suspected cases of abuse &/or neglect to the Disabled Persons Protection Commission (DPPC). PREPARATION, KNOWLEDGE, SKILLS & ABILITIES Bachelor?s degree preferred. Minimum 2 years prior experience with people with disabilities. 1 year of direct care, CNA, or human services experience preferred. Ability & willingness to provide hands-on personal care. Must be able to effectively communicate in both verbal & written form with coworkers & supported individuals & read fluently. Valid driver?s license in the state of Massachusetts required. Successful completion of all state & Agency mandated trainings required upon hire. OTHER DUTIES & RESPONSIBILITIES Attends all meetings, as requested. Performs additional duties, as assigned. WORKING CONDITIONS/PHYSICAL DEM&S Frequent standing, walking, bending, & twisting. Ability to lift up to 50 pounds. Ability to pull up or transfer individuals weighing up to 250 pounds.
