The Chart Accuracy Specialist II is responsible for maintaining the integrity of the electronic medical record to include management of chart correction processes in Health Connect (Epic) and EMPI (Electronic Master Patient Index) maintenance. Coordinates the parties involved in each chart correction case and is responsible for ensuring all necessary corrections are made and each case is validated prior to closure. Communicates with departments throughout Kaiser Permanente, partners, Health Information Exchanges, and affiliate organizations to ensure accuracy of information and coordinate EMPI changes across the enterprise. Must be able to identify and report CMS fraud related issues.

Essential Responsibilities:

  • This position, knows and complies with all Kaiser Permanente quality, safety, and emergency policies and procedures. Demonstrates quality and effectiveness in work habits and clinical practice in every interaction with patients, colleagues, providers, and leadership. Ensures patient safety in the preparation and provisioning of care related to but not limited to medications, procedures, infection prevention, fall prevention, including consistent use of two patient identifiers and procedural time outs. Reports safety hazards, accidents and incidents, and unsafe working conditions promptly.

  • Utilizes the EHR (EPIC) Workspace tools and other chart correction tracking tools to report, inventory and measure timeliness of completion for chart error corrections.

  • Tracks, reports, and communicates on all departments, partners, Health Information Exchanges and affiliate organization to ensure their systems and databases are completed and in sync with Epic (HealthConnect) within a timely manner.

  • Provides customer service to Kaiser Permanente members and staff to ensure that incorrect data is identified, charting errors are communicated to the parties identified in the chart correction policy, erroneous data is corrected, and is verified that the clinical information was updated completely and correctly.

  • Works within the Membership Administration System (Common Membership) to assure accuracy and synchronization within Health Connect.

  • Further researches isolated chart correction to ensure other areas of member record were not affected.

  • Uses trends and data on chart correction errors to develop targeted education to relevant audience of providers, clinicians and other KP employees. Also informs them on how to report chart correction issues.

  • Analyzes reports to identify possible Enterprise Master Patient Index (EMPI) compromises and reports potential cases to EHR Chart Accuracy Analyst/Coordinator for further evaluation.

  • Sets up Care Everywhere (HIE - Health Information Exchange) links with EPIC EHR affiliates.

  • Conducts Care Everywhere (Health Information Exchange) extractions with EPIC EHR affiliates.

  • Links and extracts Health Information electronically using HIE functionalities with non-EPIC EHR affiliates.

  • Monitors demographic change reports and works with Membership for remediation.

  • Reports production and turnaround time.

  • Processes the internal Data Integrity/Chart Accuracy e-mail.

  • Processes EPICs Data Integrity/Chart Accuracy In-basket e-mail.

  • Conducts the misfiles process for scanned documents.

  • Performs the monthly CMS Age-in process by merging relevant fields within HealthConnect CIS Viewer, and coordinates with the Chart Accuracy Specialist I to re-label the records.

  • Researches and merges duplicate patients in the EHR and provides communication regarding merged records to KP Ancillary.

  • Assists in tasks of a Chart Accuracy Specialist I as assigned.

  • Destroys paper medical records that meet State, Legal and KP policy requirements and generates a Certificate of Destruction.

  • Assists Chart Accuracy Specialist I as needed in processing electronic (Right Fax) faxes.

  • Processes customer service calls.

  • Works in a paperless environment.

  • Performs other duties as assigned.

    Notes:

  • Associate Degree, RHIT Certification preferred or minimum of 5 years of relevant experience.

    Basic Qualifications:

    Experience

  • Minimum of five (5) years of experience working in the healthcare field required, of which at least two (2) years of experience was spent working with electronic medical records doing review and/or audits.

    Education

  • Associates degree in health information technology (HIT) AND a current Registered Health Information Technology (RHIT) certification, OR associates degree in health information technology (HIT) AND will obtain the Registered Health Information Technology (RHIT) examination within department guidelines, OR associates degree in health care related field AND current Coding Certification OR current Certified Medical Transcriptions Certification (CMT), OR high school diploma OR General Education Diploma (GED) AND will obtain an associates degree in Health Information Technology AND pass the Registered Health Information Technology examination within department guidelines, OR high school Diploma OR General Education Diploma (GED) AND a current Coding Certification OR current Certified Medical Transcriptions Certification (CMT) AND will obtain an associates degree in a Healthcare related field within department guidelines.

    License, Certification, Registration

  • Medical Transcriptionist Certificate OR Coding Associate Certificate from American Health Information Management Association OR Registered Health Information Technician Certificate within 6 months of hire from American Health Information Management Association

    Additional Requirements:

  • Comprehensive knowledge of medical terminology.

  • Demonstrated ability to work independently with minimal supervision.

  • Strong ability to review and recommend enhancements.

  • Strong ability to readily identify more efficient ways of doing business.

  • Willingness to be flexible depending upon department and scheduling needs.

  • Basic completion of a Medical Terminology course.

  • Working knowledge of HIPPA.

  • Ability to utilize the terminal digit order methodology.

  • Basic Microsoft Office skills.

  • Basic Adobe PDF skills which includes splitting, adding, deleting, editing and writing documents.

  • Ability to analyze various reports.

  • Ability to work with and maintain confidentiality of physician, and all patient, account and personnel data.

  • Ability to prioritize workload, meet deadlines, and take on additional projects.

  • Strong interpersonal and excellent written and verbal skills.

    Preferred Qualifications:

  • Minimum seven (7) years of experience in the healthcare field preferred, of which at least four (4) years of experience was spent working with electronic medical records doing review and/or audits.

  • Experience using Web based applications including Common Membership preferred.

  • Bachelors degree in HIM or Healthcare related field preferred.

  • Registered Health Information Administrator (RHIA) designation, LVN, or healthcare related certification preferred

  • Knowledge of KP Legacy Systems preferred.
