UCHealth Clinical Social Worker CM in Aurora, Colorado
Assesses patients and/or patients' families, caregivers, and/or legal representatives and arranges for needed interventions. Plans interventions to help patients cope with social, emotional, economic, and environmental problems and provides short-term treatment planning for mental, emotional, and behavioral disorders, conditions, and addictions.
We are looking for a full-time LCSW program coordinator to help develop and lead a new interdisciplinary, cross-campus team to help improve our care of victims and survivors of elder abuse and neglect. This person will help not only to manage the day-to-day activities of the newly formed Vulnerable Elder Services, Protection, and Advocacy Team (VESPA) at the University of Colorado Division of Geriatrics but will also help to develop the clinical programs of this team, the research infrastructure, and outreach provided. We are looking for someone with either a background in geriatrics, aging services, victim advocacy, family violence, or someone with a passion for caring and advocating for older adults and their families. This role entails both clinical and administrative duties.
Work Schedule: Full-Time Days
Participates in case reviews to evaluate care management and progress toward goals. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
Periodically administers and interprets tests and measures of psychosocial functioning for the purpose of diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities, implementing an appropriate assessment-based treatment plan.
Supervises and/or trains new staff, lower-level social workers, students or interns.
Within the scope of the job, requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Other information:Minimum Required Education: Master's degree in Social Work. Required Licensure/Certification: State licensure as a Licensed Clinical Social Worker (LCSW). Minimum Experience: None Required.
BLS through the American Heart Association or theAmerican Red Cross CPR for the Professional Rescuer with the card in hand before the start date. BLS or CPR card must be good through sixty days ofhire. All life support certification cards must be issued by eitherthe American Heart Association or the American Red Cross CPR for theProfessional Rescuer. (American Red Cross for the Professional Rescuer is onlygood upon hire and all renewals will need to be through an American HeartAssociation sanctioned course.)

The pay range for this position is: $29.54 - $44.31/ hour . Pay is dependent on the applicant's relevant experience.
An annual bonus may be paid to eligible employees based upon organizational and individual performance.
